Has one of your qmail users reported a missing e-mail?
Maybe they sent it, never got a bounce message, but the recipient says they never got it.
One of the things to check is your mail logs to see if the remote mail server accepted the e-mail.
The long line will contain their IP address and something like Remote_host_said:_250_OK.
If they accepted it, but did not deliver it, you'll want to have your mail contact ask the local e-mail support group what happened to the e-mail after it was accepted.
