blocklist

Hello there, I have noticed this message in my mail server's logs and I have tried to unlist my IP from their services. The web page seems to be a catch 24 : Step 3: Send a Removal Request There's not form and not send button. Anyone knows how to ask for unlisting from this blocklist?
