Is your qmail relay rejecting a sender with the following error?
553 sorry, that domain isn't in my list of allowed rcpthosts (#5.7.1)
If this is an SMTP relay, check your IP allow list and make sure the senders IP address is allowed to relay.
If it's an SMTP relay that requires authentication verify that you have your username and password configured in your outgoing mail server settings.
If this is an MX server, check that the destination domain is configured as local on your mail server.
